
Your First Appointment
Once you arrive at your therapist’s office in Palm Beach Gardens, have a seat in the waiting room. You’ll find self-serve refreshments available for your enjoyment. If the inner door is closed, we are with another client or are temporarily away from our desk. Please know that we are expecting you and will be with you shortly. We are really looking forward to meeting you and getting you under way to your peace, healing, and fulfillment!

After your appointment is confirmed, you will be sent an email containing a link to your client portal. We will send you several documents to electronically sign as well as an intake form. It is important for you to complete these documents prior to your appointment so we can get to know you before you arrive, as well as make sure your client record is complete.
If you are the parent or guardian of an adolescent or child therapy client, there is parent intake and other forms to sign. Teenage clients may be expected to fill out their own Intake Form, unless there is a reasonable reason they are unable to do so.
One hour goes by fast in a counseling session. We're sure you want to spend as little of that time as possible doing paperwork. Yuck! So fill out the forms, send them back through the client portal, and we will be able to review your personal history more quickly so we can start the healing process as soon as possible.

If you would like us to coordinate care with another provider (your psychiatrist, primary care physician, legal representative, insurance provider, etc.), please complete the Release of Information form to authorize release of psychotherapy information.
